# RCA Reason & Description

Define root cause analysis reasons and related descriptions.

## Overview

The **RCA Reason & Description** module is used to configure the classifications available during root cause analysis (RCA). It allows you to define and manage the following:

* **RCA Reasons**
  * Create and maintain the primary causes or triggers used to categorize RCAs.
* **RCA Descriptions**
  * Create and maintain descriptions associated with each RCA reason to further classify and document issues.

## How Settings Are Applied

Reasons defined in this module appear in the **RCA Reason** dropdown within relevant applications. When a reason is selected, the associated description(s) become available in the **RCA Description** dropdown.

## Configuring RCA Reasons

Use this section to create and manage RCA reasons.

### Defining Reasons

Click **Add** to create a new reason. A new row will appear in the grid.

* **Reason:** Enter a descriptive name for the reason.
* **Reason Details:** Provide additional information or context describing the purpose and intended use of the reason.

### Managing Reasons

Once a reason is defined, the following actions are available:

* **Edit:** Click directly in a grid cell to modify its contents.
* **Delete** (<i class="fa-trash-can">:trash-can:</i>): Click to remove a reason.

Confirm deletion by clicking **Save Changes** or clicking outside the grid. To cancel, click **Discard Changes** or **Undelete** ( <i class="fa-xmark">:xmark:</i> ).

### Saving & Discarding Changes

After making changes, choose one of the following:

* **Discard Changes:** Click to revert any unsaved modifications.

Use this section to create and manage RCA descriptions associated with each reason.

### Defining Descriptions

Click **Add** to create a new description. A new row will appear in the grid.

* **Reason:** Select the RCA reason associated with the description.
* **Description:** Provide a description of the specific issue, condition, or circumstance associated with the selected reason.

### Managing Descriptions

Once a description is defined, the following actions are available:
