Review the completed action to ensure all required activities were performed, documented appropriately, and supported with sufficient evidence. Consider the following:

<table data-card-size="large" data-view="cards"><thead><tr><th></th><th></th></tr></thead><tbody><tr><td>✅ <strong>Quality of Execution</strong></td><td>Was the action implemented correctly and in accordance with the defined requirements, procedures, or expectations?</td></tr><tr><td>✅ <strong>Findings &#x26; Supporting Evidence</strong></td><td>Are the documented findings, outcomes, and uploaded evidence clear, relevant, and sufficient to demonstrate completion of the action?</td></tr><tr><td>✅ <strong>Effectiveness</strong></td><td>Does the documented evidence demonstrate that the implemented action was effective in resolving or mitigating the issue?</td></tr><tr><td>✅ <strong>Remaining Risks or Gaps</strong></td><td>Are there any unresolved concerns, remaining risks, or additional follow-up actions required?</td></tr></tbody></table>

***

{% endstep %}

{% step %}

### Provide Feedback

Use the available fields to communicate your assessment.

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

* **General Comment:** Enter overall feedback regarding the execution, findings, effectiveness, or quality of the completed action.

{% hint style="danger" %}
If rejecting the action, this field becomes mandatory and must include a reason.

* Your comment will be visible to the action item owner in the [Action Refused by Reviewer](/interfacing-help-files/quality-managers/action-item-application/creating-and-managing-action-items/action-refused-by-reviewer-re-execute-the-action.md) form.
  {% endhint %}

* **Comments:** Add clarifications, questions, or discussion points to support collaboration.

## Querying This Documentation
If you need additional information that is not directly available in this page, you can query the documentation dynamically by asking a question.

Perform an HTTP GET request on the current page URL with the `ask` query parameter, and the optional `goal` query parameter:

```
GET https://interfacing.gitbook.io/interfacing-help-files/quality-managers/action-item-application/creating-and-managing-action-items/sign-off-on-action.md?ask=<question>&goal=<endgoal>
```

`ask` is the immediate question: it should be specific, self-contained, and written in natural language.
`goal` is optional and describes the broader end goal you are ultimately trying to accomplish on behalf of the user. GitBook uses it to tailor the answer towards what is most useful for that goal.

The response will contain a direct answer to the question and relevant excerpts and sources from the documentation.

Use this mechanism when the answer is not explicitly present in the current page, you need clarification or additional context, or you want to retrieve related documentation sections.
