# Check Effectiveness & Approve

Verify that actions were executed as planned and achieved their intended outcome.

## Workflow Overview

<i class="fa-location-dot">:location-dot:</i> **Current Task:** Check Effectiveness & Approve

<i class="fa-user">:user:</i> **Performed By:** Assigned Eff. Check Responsible (e.g., QA Analyst, Department Manager, Compliance Officer)

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

***

## Accessing the Task <a href="#accessing-the-task" id="accessing-the-task"></a>

The **Check Effectiveness & Approve** task can be accessed through any of the following:

* The **To Do** module.
* The **Notification Panel** (via the **Notification Bell**).
* The **direct link** provided in the email notification.

***

## Completing the Task

After all associated actions have been completed and approved, assess the overall effectiveness of the CAPA. Confirm that it has been properly implemented and achieved its intended outcome. Based on your evaluation, approve the actions or return them for re-execution as needed.

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

{% hint style="success" %}
For guidance on common form features and functions, see [**Working with Forms**](/interfacing-help-files/end-users/exploring-apps/working-with-forms.md).
{% endhint %}

{% hint style="info" %}

#### Saving Your Work

To save progress without submitting, click **Save** in the top-right corner of the panel.

* The task remains in the **To Do** module for you to revisit and complete later.
  {% endhint %}

{% step %}

### Assess Implementation of CAPA

Ensure each implemented action meets defined requirements. Consider the following:

<table data-card-size="large" data-view="cards"><thead><tr><th></th><th></th></tr></thead><tbody><tr><td>✅ <strong>Effectiveness</strong></td><td>Does the action effectively address the root cause or achieve the intended improvement?</td></tr><tr><td>✅ <strong>Compliance</strong></td><td>Was the action performed according to the defined procedure or action plan?</td></tr><tr><td>✅ <strong>Residual Risk</strong></td><td>Are there any remaining risks, gaps, or nonconformities that need attention?</td></tr></tbody></table>

{% hint style="info" %}

#### Reviewing Action Execution History

The **Associated Action Items** grid shows key details about each action.

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

* To see how many times an action item has been rejected, check the **Times Rejected** figure:
  * If an item is executed only once, this field will remain blank.
  * If an item is rejected during [execution](/interfacing-help-files/quality-managers/action-item-application/creating-and-managing-action-items/execute-action.md) or sent for re-execution in this form, however, the number will increase by 1.
    * Users can re-execute an item however many times they deem necessary.
* To see the details of how an action item was executed, select the **View** ( ![](https://1488562728-files.gitbook.io/~/files/v0/b/gitbook-x-prod.appspot.com/o/spaces%2FTx7AmE0d2Q4ja20EYjF5%2Fuploads%2FIAlpvjgSBR3lKZjwsXm7%2FIMS-KI-L-9.svg?alt=media\&token=262f3701-b544-43fe-b1bc-d17a29a18e55) ) icon.
  {% endhint %}

{% endstep %}

{% step %}

### Conduct Effectiveness Check

Perform an effectiveness check to confirm that the CAPA achieved its objectives.

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

* **Check Effectiveness Notes:** Enter notes on your evaluation of the implemented actions.

{% hint style="danger" %}
If rejecting an action, this field becomes mandatory and must include a reason.

* Your notes will be visible to the action item owner in the [**Execute the Action**](/interfacing-help-files/quality-managers/implementing-capas-apps/navigating-the-capa-management-workflow/execute-action-s.md) form.
  {% endhint %}

* **Effectiveness Check Result:** Provide your assessment of the CAPA’s effectiveness, describing the evaluation process and outcomes.

* **Effectiveness Files:** Upload supporting documents or evidence to substantiate your assessment.

***

{% endstep %}

{% step %}

### Mark Actions to be Re-Executed

If needed, flag actions for re-execution

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

* **Select the Actions:** Select the actions to be re-executed.
* **Set New Due Date:** Select a new due date for the chosen action item(s).
* **Mark Selected Actions For Re-Execution:** Click to confirm.

  * All marked actions appear in the **Actions Marked for Re-Execution** grid.

  <p align="center"><kbd><strong>IMAGE</strong></kbd></p>

  * To edit the assigned owner or due date, click inside the corresponding column.
  * To delete an action, select the **Delete** ( <i class="fa-trash-can">:trash-can:</i> ) icon.

  <div data-gb-custom-block data-tag="hint" data-style="info" class="hint hint-info"><p>Confirm deletion by clicking <strong>Save Changes</strong> or clicking outside the grid. To cancel, click <strong>Discard Changes</strong> or <strong>Undo</strong> ( <i class="fa-rotate-left">:rotate-left:</i> ).</p></div>

***

{% endstep %}

{% step %}

### Adjust Key Dates

Set or modify key dates related to CAPA closure and effectiveness verification:

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

* **New Closure Date:** Select or modify the revised date by which the CAPA is expected to be completed, if the original date cannot be met for any reason.

{% hint style="danger" %}
If rejecting an action, this field becomes mandatory.
{% endhint %}

* **Effective Closure Date:** Select or modify the date when the CAPA is fully implemented and confirmed effective.
* **Eff. Start Date:** Select or modify the start date for the effectiveness check period.
* **Eff. End Date:** Select or modify the end date for the effectiveness check period.

***

{% endstep %}

{% step %}

### Provide Feedback

Use the **Comments** field to share feedback, ask questions, or provide clarification.

<p align="center"><kbd><strong>IMAGE</strong></kbd></p>

{% hint style="info" %}

#### Commenting Guidelines

* You may **reply** to others' comments but cannot edit or delete them.
  {% endhint %}

***

{% endstep %}

{% step %}

### Finalize Assessment

From the **Task Resolution** dropdown, select one of the following actions:

* **Approve All Action(s)**

  Approve all actions, indicating that they have been satisfactorily completed.

  → Next Step: [Sign Off on CAPA](/interfacing-help-files/quality-managers/implementing-capas-apps/navigating-the-capa-management-workflow/sign-off-on-capa.md)

{% hint style="danger" %}
Do not select this option if any actions are marked for re-execution; otherwise, re-execution will not occur.
{% endhint %}

* **Reject Action(s)**

  Reject the selected action(s), returning them to their assigned owner(s) for re-execution.

  → Next Step: [Execute Action(s)](/interfacing-help-files/quality-managers/implementing-capas-apps/navigating-the-capa-management-workflow/execute-action-s.md)

{% hint style="danger" %}
Before submitting, enter a **New Closure Date** and provide a comment in the **Check Effectiveness Notes** field.
{% endhint %}

## Querying This Documentation
If you need additional information that is not directly available in this page, you can query the documentation dynamically by asking a question.

Perform an HTTP GET request on the current page URL with the `ask` query parameter, and the optional `goal` query parameter:

```
GET https://interfacing.gitbook.io/interfacing-help-files/quality-managers/implementing-capas-apps/navigating-the-capa-management-workflow/check-effectiveness-and-approve.md?ask=<question>&goal=<endgoal>
```

`ask` is the immediate question: it should be specific, self-contained, and written in natural language.
`goal` is optional and describes the broader end goal you are ultimately trying to accomplish on behalf of the user. GitBook uses it to tailor the answer towards what is most useful for that goal.

The response will contain a direct answer to the question and relevant excerpts and sources from the documentation.

Use this mechanism when the answer is not explicitly present in the current page, you need clarification or additional context, or you want to retrieve related documentation sections.
