# Execute Risk Assessment

Complete the assigned risk assessment.

## Accessing the Task <a href="#accessing-the-task" id="accessing-the-task"></a>

The **Execute Risk Assessment** task can be accessed through any of the following:

* The **To Do** module.
* The **Notification Panel** (via the **Notification Bell**).
* The **direct link** provided in the email notification.

## Completing the Task <a href="#completing-the-task" id="completing-the-task"></a>

When the **Start of Assessment Period** date is reached, or if the assessment is triggered immediately, complete the risk assessment.

### Complete Questionnaire

Complete the questionnaire assigned to the risk assessment, if available.

* Enter your response in the window that opens.
* Use the navigation buttons as needed:

  * **Previous:** Click to save your response and return to the previous question.
  * **Save and Close:** Click to save your response and close the window.
  * **Next:** Click to save your response and proceed to the next question.

### Document Analysis Findings (Optional)

Document any analysis performed in support of the risk assessment.

* **Summary of the Analysis:** Provide a high-level summary of the analysis performed, including key observations, findings, and conclusions.
* **Provide Analysis Documentation:** Upload supporting files, reports, evidence, or other documentation relevant to the analysis.
* **Analysis Tools:** Select the analysis tools used to support the assessment.

Use this option to create a visual representation of the risk analysis.

If selected, the free draw control, a digital tool for creating freehand drawings, will appear.

Use this option to conduct a SWOT analysis.

If selected, fields for **Strengths**, **Weaknesses**, **Opportunities**, and **Threats** will appear, enabling you to document each component.

Use this option to perform a Failure Modes and Effects Analysis (FMEA).

If selected, the following grid will appear:

Click **Add** to create a new entry. A new row will appear in the grid.

* **Process Step:** Specify or describe the process step being analyzed.
* **Failures:** Identify potential failure modes or ways in which the process step could fail.
* **Effect:** Describe the potential effects or consequences of each failure mode.
* **Cause:** Identify the root causes or reasons behind each failure mode.
* **Severity:** Rate the severity of each failure mode.
* **Occurrence:** Rate the likelihood of each failure mode occurring.
* **Detection:** Rate the likelihood of detecting each failure mode before it causes an impact.

The **Severity**, **Occurrence**, and **Detection** ratings are typically assessed on a scale of 1 to 10.

Upon saving the row, the **Risk Priority** and **Criticality** values are automatically calculated based on the **Severity**, **Occurrence**, and **Detection** ratings assigned to each failure mode.

Equations:

* Risk Priority (RP) = Severity (S) × Occurrence (O) × Detection (D)
* Criticality (C) = Severity (S) × Occurrence (O)

These values determine the overall prioritization (RP) and criticality (C) of each failure mode.

### Identify & Evaluate Risks

Identify risks associated with the assessment scope and evaluate their potential impact, likelihood, and detectability.

Click **Add** to create a new entry. A new row will appear in the grid.

* **Risk Name:** Enter a name that clearly identifies the risk.

Once a risk is added, the following actions are available:

* **Edit** ( <i class="fa-pen">:pen:</i> ): Click to evaluate or analyze the identified risk.

The **Risk Analysis** form allows you to document and evaluate an identified risk.

1. **Define Risk**

Provide general information about the identified risk.

* **Risk Name:** Update the name of the identified risk.
* [**Risk Type:**](#user-content-fn-1)[^1] Select the category that best describes the identified risk.
* [**Risk Matrix:**](#user-content-fn-2)[^2] Select the risk matrix that will be used to evaluate the risk.

Upon selection, the options inside the **Impact**, **Probability**, and **Detectability** dropdowns are automatically updated to reflect the parameters of the chosen risk matrix.

* **Description of the Risk:** Provide a detailed description of the identified risk, including relevant context and concerns.

2. **Document Causes & Effects**

Use the **Cause(s) and Effect(s)** grid to document the causes and effects associated with the identified risk.

* Click **Add**.
* In the newly added row:
  * **Cause:** Specify the cause contributing to the identified risk.
  * **Effect:** Describe the potential effect or consequence resulting from the identified risk.
* Click **Save Changes** or click outside the grid to save.
  * To cancel, click **Discard Changes**.

#### Managing Causes & Effects

To remove a cause-and-effect entry, click the **Delete** ( <i class="fa-trash-can">:trash-can:</i> ) icon.

* Confirm deletion by clicking **Save Changes** or clicking outside the grid. To cancel, click **Discard Changes** or **Undelete** ( <i class="fa-xmark">:xmark:</i> ).

3. **Evaluate Risk**

Assess the risk using the selected risk matrix.

* **Impact:** Select the value that best represents the potential consequence of the risk.
* **Probability:** Select the value that best represents the likelihood of the risk occurring.
* **Detectability:** Select the value that best represents the likelihood of detecting the risk before it results in an impact.

The **Score** is automatically calculated based on the selected **Impact**, **Probability**, and **Detectability** values.

Upon saving the form, the calculated score automatically populates the corresponding row in the grid within the **Risk Identification & Evaluation** accordion.

4. **Register Risk**

Specify where the identified risk will be stored.

* **Select the Folder:** Select the folder where the identified risk will be added or stored.
  * Instead of a dropdown menu, this field opens a form.
    * In this form, navigate through the tree structure displaying risk folders. To select a folder, simply double-click it.
  * The selected folder name will populate the **Folder Name** field.
* **Identifier:** Enter a unique identifier for the risk.

5. **Save Changes**

When ready to save, choose one of the following options:

* **Previous:** Click to save your changes and return to the previous risk.
* **Save and Close:** Click to save your changes and close the form.
* **Next:** Click to save your changes and proceed to the next risk.

Upon saving the **Risk Analysis** form, the **Score** field in the grid automatically updates to display the calculated score based on the selected impact, probability, and detectability values.

### Define Actions

Define actions intended to mitigate, control, eliminate, or otherwise address identified risks.

#### Actions & Their Impact

An **action item** represents a corrective, preventive, or improvement activity intended to achieve measurable outcomes in quality, compliance, or risk reduction.

* When defined, actions are formally tracked through the workflow. They are assigned to responsible owners and must be completed and approved before the report can be closed.

Click **Add** to create a new action item.

The **Add Action Item** form allows you to create actions associated with the risk

This form is a modal version of the standard **New Action** form found in the **Action Items** application. It retains nearly all the original fields and functionality, but is adapted to fit the context of the current application.

**Key Differences:**

* **Source of the Action:** This field is automatically populated based on the current application context and cannot be modified.
  * For example, in this case, the source is set to **Risk**.
* At the bottom of the form, you can choose from the following save options:
  * **Save and New**: Click to save the action and open a new form to create another.
  * **Save and Close**: Click to save the action and close the form.

From the **Task Resolution** dropdown, select **Ready for Review**, then click **Submit**.
